Other Manufacturer's Forms
Our measuring system and forms are interchangeable with those used by other major manufacturer's and the IMF system. We gladly accept orders submitted on forms other than our own. Authorization to substitute is not required.
All orders require a prescription, regardless of the form used.

Printing PDFs
The default printer setting for some versions of Adobe Acrobat Reader reduce (shrink) the forms. In some instances (such as form B-1A-W) maintaining the form at 100% is critical for proper engineering of the product being ordered. Please check and confirm your settings before printing.
